A: The item is just as you see it in the picture, no adapter or power supply is included.

 

One Balun will connect directly to one of your DVR BNC connections and the other balun at the other end of your CAT5 cable may need a BNC "female" coupler to connect it to your camera or Camera extention cable depending on what your camera has on it.
